Physicians and Medical Clinics

Physicians and Medical Clinics CLICK HERE for the Links for Life Application.  Please complete the Doctor’s portion, have the patient complete and sign the Patient Portion, and return it to our office.  Completed applications can be emailed to staff@linksforlife.org  or  faxed to (661) 322-5655. 

Every Monday, Links for Life Volunteer Screeners review completed applications that have been returned to our office.  After review, approvals are faxed back to the requesting physician with a list of Radiologists that accept payment from Links for Life.

Please provide Approval Forms to the patient who will then be able to make an appointment at an approved Radiologist to have the procedure.  The bill will come directly to Links for Life and results will be returned to the requesting physician’s office.
